Sadly at the time of writing, I can only point out a few inconsistencies in the report as I cannot read much of it. I called the DWP and they could not help with the 1st 3 sections which I asked about and so they have sent it away to be typed up. However this did not stop them making a decision based on its contents !!!
For example, at the assessment, the doctor commented on the walking aids which my partner uses and she replied by stating she were given to her by the physiotherapy department at Northwick Park Hospital. In 2 sections of the report it states that she does not use walking aids!!
Much of her medication is missing from the report (and associated side effects) despite the medication being given to and handled by the doctor.
The report also states that my partner “refused” to perform some medical tasks such as bending. My partner clearly said “I would rather not because of the pain and likely need to go to the toilet if I do, but I will do it if you need”. The doctor clearly replied “No, that is ok”. It is very upsetting for her to read that this is a “refusal”
